If this questionnaire is not completed and submitted prior to your appointment, or is submitted more than 24hours before your appointment, you may have to re-complete the form and submit or your appointment may be re-scheduled.
COVID-19 BUSINESS UPDATE
As of THURSDAY 24TH SEPTEMBER 2020, ALL clients will be required to download the 'NHS COVID 19 APP' onto their mobile phone and use the app to scan my business QR code on arrival at the salon.
This is an additional step in the Track & Trace programme that MUST be followed.
The app needs to be downloaded via Play Store or Google Play BEFORE arrival at the salon.
